The Problem: One Immune Response, Many Devastating Injuries

Most people think of a stroke or heart attack as a single event. In reality, the initial emergency is only the beginning. In the hours and days that follow, the body mounts an immune response that becomes destructive - mobilizing immune cells that flood into the injured tissue and cause a second wave of damage after the original injury.

The striking reality is that this is the same fundamental response across different injuries:

1. After a stroke or traumatic brain injury, the brain can swell so severely it crushes itself against the skull and drive long-term neurological damage.
2. After a heart attack, returning blood flow triggers inflammation and scarring that increases risk of heart failure.
3. After a serious orthopedic injury, the same runaway inflammation worsens tissue loss and recovery.

Millions of patients worldwide are affected each year, and no approved therapy targets the root cause. Existing treatments only manage symptoms.

The Science: An Established Problem, a New Solution

For decades, researchers have understood the outline of this problem. After a serious injury, immune cells surge out of the bone marrow, enter the bloodstream, switch into an aggressive inflammatory state, and infiltrate the damaged tissue - peaking within 48 hours and driving much of the lasting damage.

For just as long, clinical programs tried to solve this problem by blocking these immune cells from reaching the injury in the first place. Every one of these programs failed. The reason turned out to be a crucial insight - those same immune cells are also needed for repair. Keeping them out doesn't just stop the harm; it stops the healing.

Our co-founders at Stanford and Johns Hopkins identified the missing piece. The problem isn't that these cells are infiltrating the tissue - it's the state they're in when they arrive. They identified a molecular switch on these cells that, when activated by a naturally occurring protein, reprograms them from a destructive state to a protective, reparative one. Rather than blocking the immune response, this approach redirects it.

Two findings make us confident this is real, not just theory:

1. Human data: In patients with brain injury, the harmful immune cells carry a specific marker - and the more of them a patient has, the more severe the brain swelling. The biology we're targeting is measurable in real human injury, not just in the lab.
2. Preclinical proof of concept: Activating this switch reprogrammed the immune cells, significantly reduced swelling, and improved survival and recovery across multiple injury types.

This work is backed by published research in leading peer-reviewed journals, including Brain, Behavior, and Immunity; Circulation; and Neurosurgery.

Our Solution: A First-in-Class Therapy to Enhance Recovery After CNS, Cardiac, and Orthopedic Injuries

EGT-101 is a next-generation version of this natural protein, engineered for clinical use. It was designed with two critical features:

1. Precision without broad immunosuppression. EGT-101 activates the protective switch without broadly shutting down the immune system. In our complete Phase 1 trial in healthy volunteers, we saw no dose-limiting toxicities or serious adverse events, and no changes to patients' T-cell counts.
2. One dose, full coverage. With an extended half-life of roughly one week, a single infusion covers the secondary damage window - no repeated dosing in an ICU setting.

We are now ready to launch our first trial in patients.

Our Clinical Development Plan: A Proof-of-Principle Trial in a High-Need Population

Our first target is malignant cerebral edema after large vessel occlusion stroke (MCE-LVO) - one of the most devastating conditions in medicine.

Large vessel strokes affect roughly 80,000 US patients each year. Many suffer significant brain swelling, and about 15% progress to malignant cerebral edema. The only life-saving solution for MCE is rescue surgery to remove a portion of the skull and relieve the pressure. Without surgery, over 70% of these patients die; even with surgery, 30% still die and nearly half are left severely disabled. Each patient can generate $150,000-$300,000 or more in ICU, surgical, and long-term care costs. There is no approved drug for MCE.

We chose this indication deliberately. These patients are under continuous ICU monitoring throughout their hospital stay, which lets us generate clear data on both safety and early biological effect. We are launching a Phase 1b double-blind, placebo-controlled trial. Because outcomes in these patients are often stark - many progress to massive brain herniation and rescue surgery - even an early trial can show whether EGT-101 reduces that progression. Those are ICU-period signals that can point to better long-term outcomes.

The Market: A Premium Market with Platform Upside

We think about our market opportunity in two layers:

1. The beachhead we can defend. MCE-LVO alone is a premium market with an established reimbursement path. At a realistic penetration of the addressable population and a premium price justified by the surgical and long-term care costs it offsets, we project peak sales approaching $1 billion from this single indication - distributed through roughly 300 comprehensive stroke centers that see a majority of large vessel stroke patients.
2. The platform on top: Once we de-risk the approach in MCE-LVO, the same immune biology opens the door to a much larger opportunity: heart attack, traumatic brain injury, orthopedic injury, battlefield trauma, and sports medicine. The mechanism that drives secondary swelling after a stroke is implicated in other injury types. We believe EGT-101 can address them all.

Building Toward Long-Term Protection

1. EGT-101 Ownership: We hold a global exclusive composition-of-matter license to the EGT-101 technology with South Korean biotech Genexine, Inc., and we have agreed to terms for full ownership transfer.
2. First-Mover Position: We are pursuing first-in-class coverage for this mechanism across multiple injury types.
3. Long-Term Exclusivity: We hold global exclusive rights extending through 2041, with a continuation filing strategy designed to extend protection beyond the base patent term.

$7.5M to Reach Our First Critical Inflection Point

We are raising $7.5 million to execute our clinical development plan through Phase 1b readout in 2H 2027. This is the milestone that matters. Based on direct feedback from pharma contacts, Phase 1b safety and biomarker data is the threshold for serious licensing and M&A conversations. It also de-risks the broader platform - opening the door to pipeline expansion, DoD contracts, and athletic organization partnerships.

Egret was co-founded by Dr. Michael Lim (Chair of Neurosurgery at Stanford), Dr. Christopher Jackson (Associate Professor of Neurosurgery at Johns Hopkins), and Dr. Daniel Chai (Managing Partner of Turret Capital). Vedik Navale, our Chief of Staff, brings biotech operating experience.
