Memo

The Problem Is Enormous

Every year, ~131,000 Americans are diagnosed with end-stage kidney disease and require dialysis. The gold standard solution — an arteriovenous fistula (AVF) — fails to mature in up to 60% of patients, forcing them onto catheters that carry life-threatening infection risk and driving $2.8 billion in annual U.S. healthcare costs. There is currently no approved pharmacological therapy to fix this.

Our Solution: Bionanomatrix

Endomimetics has engineered a first-in-class Vascular Access Gel that is applied at the time of AVF surgery. Built on our proprietary Bionanomatrix platform — self-assembling peptide nanostructures licensed from UAB — the gel releases nitric oxide locally to promote vessel healing, reduce inflammation, and prevent failure. Preclinical results are striking:

  1. >70% reduction in venous intimal hyperplasia vs. controls
  2. Improved vascular remodeling and endothelialization
  3. No direct competitors on the market

Traction & Validation

We don't just have a great idea — we have proof and momentum:

  1. $2.8M Phase II SBIR from NIH (brain aneurysm application, 2024)
  2. $292K + $100K SBIR/NIH grants for orthopedic applications (2023–2024)
  3. Research collaboration with BISCO, Inc. for dental applications
  4. Global patent portfolio through 2043 across US, EU, Japan, Australia, and more
  5. Published in top journals: Biomaterials, Acta Biomaterialia, Circulation

A Platform, Not Just a Product

The AVF Gel is just the beginning. Bionanomatrix is a multi-indication platform with a combined TAM exceeding $21 billion:

Application Market Size

Cardiovascular Stent Coatings $12.6B

Angioplasty Balloon Coatings $2.8B

Dental Pulp Capping $2.4B

Orthopedic Gel $2.0B

Flow Diverter Coatings $550M

Endomimetics operates as an IP Foundry — de-risking each program at the platform level, then spinning validated assets into dedicated new companies for licensing/exit. This creates multiple independent value events for investors from a single investment.

The Team Has Done This Before

  1. Joseph Garner, PhD (CEO) — Successful exits from 4 companies, including Soluble Therapeutics (2016)
  2. Brigitta Brott, MD (CMO) — UAB Interventional Cardiology; SCAI Standards & Guidelines Committee
  3. Ho-Wook Jun, PhD (CSO) — AIMBE Fellow, American Heart Association Fellow, UAB Engineering Innovation Fellow
  4. Advisors include veterans from Surmodics, Frenelle Pharma, and a VC board member whose prior investment (Relypsa) was acquired for $1.53B

The Path Forward

Milestone Target

IND-enabling studies scheduled to begin Q3 2026

FDA IND submission (AVF Gel) Q4 2027

Phase I/II first-in-human trial Q1 2028

Phase III & commercial launch 2029–2030+

Invest Now

We are raising a $3M SAFE to fund platform-wide FDA IND-enabling studies. Investors receive:

  1. Conversion at the lower of a valuation cap or discount to the next priced round
  2. ~30% embedded equity participation rights in each spin-out new companies
  3. Most Favored Nation (MFN) rights
  4. Standard information rights

The window to get in at the platform level — before IND submission, before clinical data, before the spin-outs — is right now.
