Answer: Each country we are working in has different opportunities and challenges. Chile, for example has been spending a LOT on hospital information systems that are most often failed implementations. Healthcare in Chile is primarily provided by state run healthcare regions and they have pretty hefty budgets for automation and go to bid on these every 3-5 years. For example, the recent bids we have participated in are over $3M in total. If we win this bid, our share will be $1.3-$1.6M with the rest of the revenue going to our partner who will provide staffing for implementation and support services (which means our costs are very low.) We will soon be bidding on a $5M+ (for us) project within the next month and are already the preferred vendor. While these are big contracts, they are sometimes very slow to come to fruition, especially in the midst of a pandemic. In Mexico, and many other LATAM countries, most of the hospital care is provided by smaller private hospitals or private hospital systems with government hospitals providing care for those who can’t afford it elsewhere. Many of the private hospitals are well funded and most government hospitals are strapped for funding, so the private market is our main focus there. Some of the projects we are working on are perpetual license contracts, but most are software as a service (SaaS). With a perpetual license, we will get paid the majority of the money in the first year with an annual service contract, which is usually 20% of the purchase price. With SaaS, we get paid monthly over a period of years. For example, one of the projects we are in first place for will pay $1.2M in year one and $200K per year for service. Another will pay $1.6M over 5 years. With private hospitals the payment systems are negotiated as part of the contract process. We are expecting a contract with a US owned non-profit hospital in the next few weeks in Mexico, which will pay $500K over 5 years. Smaller hospitals will use our HarmoniMDGO system, which is a standardized, out-of-the-box system without customization. The pricing on this system is based on their patient volume and is paid monthly as a service fee with an implementation fee for starting up. We have also built some very customized interfaces for a beauty clinic and urgent care centers in Paraguay. While these are not big money makers, they are good demonstrations of the flexibility of the platform. 2. Who are your direct competitors in US, Mexico, LATAM, India? Answer: We are not working in the US at present although we are in discussions with a very large insurance company to provide a system for home care of critically ill patients. There is very little competition in Mexico at present. Some US companies have made small inroads but their systems tend to be too difficult to adapt to the local needs and we are replacing some of these already. Cerner (US) has tried in the past to enter the Chilean market and failed and Spanish companies have tried and failed in Mexico and Chile. In Chile there are well established players like InterSystems, but they are very disliked by the Chileans and have a bad track record of failed implementations. They too suffer from a lack of flexibility to adapt to changing needs of Chilean healthcare regions. At present, we are not focused on India or Africa because we do not have the resources to develop these markets. 3. How are you better than them? If India has a local solution, why will they use HarmoniMD? Answer: The reason HarmoniMD is better in all markets is because it is not a static product, but a healthcare platform. What I mean by this is that, instead of building a single product and trying to sell it to hospitals that have very different needs, we have built a platform that allows us to rapidly design user interfaces without programming. This is the most critical value in what we have accomplished and allows accelerated development of customized user interfaces and workflows to adapt to the needs of each country and individual hospitals within them. We have not seen any competition that comes close to our wide range of functionality or our speed of implementation. Many systems take years to implement, we take just a few months. It is well known that doctors and nurses generally dislike the systems they have to work with. This is mostly because the user interfaces are not intuitive or adapted to their individual work flows. With HarmoniMD, we are able to give each role in the hospital a user interface that has been designed specifically for their workflow. This eliminates extraneous steps and simplifies their tasks. This is only possible because we invested years into the development of the platform instead of a single product. We are currently focused on LATAM because it is a blue ocean (without sharks) and a growing market that needs and wants something new. Entering the US market would take much more capital and be a much slower process. Other regions will have to wait until we have the resources to tackle them. (It takes time, money and people to enter any market with a new product.) We are running lean and mean to maximize the return on investment and minimize our burn rate and this includes having a small development team in the US (4), an implementation and sales team in Mexico and some technical staff in Paraguay. It is our goal to keep our team as lean as possible so we can get to cash flow ASAP. 4. Answer: The reasons are: • Developing a complete suite of hospital management software that includes all clinical operations, inventory, purchasing, financial management and more is a monumental task that took time and investment. Building a platform that can handle all aspects of clinical and financial management for hospitals and hospital systems was even more daunting. But this also means that the barriers to entry are very high for potential competitors. • Entering new markets with a new concept of software as a service takes time and you have to build enough credibility for a hospital CEO to trust that you can run their hospital. To accomplish this, we had to implement in some major hospitals, which we did by donating the software and implementation. They are now our sales force. • Hospitals are slow to change their management practices and it is normal for them to take more than a year to make a decision. We have been in discussion with some hospitals for over 3 years and they are only now ready to move forward. With government entities, the bid process can take up to a year from start to finish, so building a pipeline of future business has taken time. • COVID put a virtual halt to hospitals exploring new systems for the last year and a half. It is only recently that the pace of acquisition of systems has resumed and we are bidding on major projects. The good news is that countries like Chile have dramatically increased their budgets for hospital systems as result of COVID and more people are now realizing they have to have more functional management systems. Having said all of that, we are in first place on two large ($1.5M+) contracts in Chile and have a number of other pending deals. These are like dominoes in that one leads to the next. As someone who has been in the software business for many years and been the chairman of a hospital, I can say that HarmoniMD has no peers and that the markets we are working in will expand dramatically in the next 10 years.
