Contracting news! Imago Rehab has renewed its contract with Vytalize Health, an Accountable Care Organization (ACO). An ACO is a group of doctors and hospitals that teams up to coordinate care for a set group of Medicare patients. Medicare rewards the group when those patients stay healthier and cost less to treat, so the group looks for care that keeps people recovering well at home. That is exactly what we strive to deliver with HCO Neuro.Vytalize first signed with us under ACO REACH, a federal program that ends in 2026. Vytalize has now chosen to continue with us under ACO LEAD, the 10-year program that replaces REACH in 2027.Since the first contract phase, we have added business development resources to build our presence in the program and hold orientation sessions for relevant practices within Vytalize’s group. This renewal locks in a decade of access to Vytalize’s Medicare population, (hundreds of thousands of members), and it follows the same land-and-expand playbook already driving HCO Neuro’s growth: enroll patients through Medicare first, then extend into their other relationships that enhance referrals and access.That playbook has produced a 17:1 LTV to CAC ratio and 6x revenue growth over the past nine months, and we expect Vytalize to follow the same path from orientation sessions to steady patient visits and recurring revenue.Our investment round is still open exclusively to the HCO community.
