Dear friends and investors,You heard from me recently with our monthly update. Growth is happening. Contracts are signed. The momentum is real.But I found myself sitting with something that didn't fit in a paragraph. Our clinical outcomes deserved their own moment, not a line item in a broader update. And so I'm writing to you separately, because you are not just investors in a company. You are part of something that is changing people's lives in ways that numbers alone can't hold.You know we're growing. This is the story behind that growth.First, a recognition.Health Care Originals was recently named to the inaugural PurposeBuilt100, one of just 100 companies selected from thousands screened across the United States, recognized as one of America's fastest-growing mission-driven businesses. The list qualifies companies by impact and ranks them by growth. That framing matters to me, because it is exactly how we built this company. Purpose and performance are not in tension here. They are the same thing.I want to tell you what that looks like on the ground.Adults with asthma.Through our partnership with the Asthma and Allergy Foundation of America on the HEAL initiative, we are serving a cohort of adults with asthma and COPD in the Dallas-Fort Worth area. Nearly a third of them live below $50,000 in annual income or have no income at all. More than three quarters had visited the emergency room for their breathing in the past year alone. Every single one of them reported having regular trouble breathing because of their asthma. These are not people who have been well-served by the healthcare system. They are people the system has largely failed.At six months, their symptoms score on the St. George's Respiratory Questionnaire has improved by nearly double the threshold that the American Thoracic Society defines as clinically meaningful care. Their breathing is measurably, significantly better. And we are still only at the six-month mark.Kids with asthma.Steelton-Highspire is a community carrying one of Pennsylvania's heaviest pediatric asthma burdens. Children there are significantly more likely to have asthma than the county average, and nearly twice as likely to face worse outcomes and costs. The region is an EPA-designated air quality nonattainment area, and Harrisburg ranks among the top fifteen cities nationally for asthma burden.We are there. Dozens of students are enrolled in our Nightingale program, provided entirely free to the district, with ADAMM devices, licensed respiratory therapists, Penn State Health as our clinical partner, and individualized asthma action plans for every child.Within just a few months, the youngest children who had been waking up constantly because of their asthma were no longer doing so. For older children, nighttime waking from symptoms dropped dramatically. Asthma limiting school or home activity fell significantly.But I want to tell you about two little girls.One is seven years old. When she joined our program, she couldn't walk very far without her asthma limiting her. She had a lot of anxiety about it. Recently, she ran down the corridor to find our respiratory therapist Heather, to tell her that she had participated in a cheer competition. And won.Another little girl couldn't breathe normally even with simple walking to the school nurse. She can now.The district nurse told us that asthma is the single most common reason students come to her office, and the second leading cause of ambulance calls and parent pickups, after seizures. She said so many of these episodes are preventable. The school board president wrote that children who cannot breathe cannot learn, and that addressing pediatric asthma is not a peripheral concern. It is central to the mission of every school.We agree. That is why we are there.Stroke survivors.Imago Rehab is restoring function and independence in people who were told there was none left to restore.A woman whose husband suffered a brain bleed stroke wrote to us recently. His previous physical therapist had told her he would never progress any further after six months. She was skeptical of virtual therapy but willing to try anything Medicare would cover. After joining our program, his arm moved for the first time. She wrote: "I cried daily and almost lost faith in my husband's possibility for improvement, but thanks to Imago, I am now crying happy tears."A speech therapist on our team shared that her patient, since starting with us, has laughed more and smiled more than she ever had before.A patient had been taking rented medical van rides to his doctor appointments and was unable to go anywhere else since his stroke. Recently, he rode in his family car for the first time. When he started with us, he could attempt only one sit-to-stand at a time. He can now complete the five-times sit-to-stand test approaching normal range, an incredible change from where he began.A patient with cerebral palsy dressed herself for the first time in her life.A stroke survivor 3 years post-stroke can now shower independently, put on his own socks, and live with more dignity and more autonomy than he had before.I want you to sit with that for a moment. Not as investors. As human beings.Now, as investors.I told you this update was about mission. But I also want to be direct with you about something, because you deserve that clarity: mission and returns are not in competition here. They are the same thing.Every person who avoids an ER visit represents thousands of dollars in avoidable spend that payers and employers no longer pay. Every child who stops missing school because of asthma is a family with one less disrupted workday, one less ambulance call, one less crisis. Every stroke survivor who showers independently is a healthcare system that spends less on downstream dependency. Our model in respiratory, and soon in neuro is we are paid, structurally and deliberately, when people get better. That is not a coincidence. It is the architecture.This is what makes HCO different from every comparable company that has come before us. We are the only one at this stage with independently validated outcomes; the only one with a business model that runs in the same direction as the clinical outcomes; the only one with a platform that expands across disease categories without starting from scratch each time. And we are the only one that was built, from the very beginning, by someone who lived this.The PurposeBuilt100 recognized us as one of America's fastest-growing mission-driven companies.
