Abdominal Pain and Dyspepsia: 25 years ago, Anand Sahai and a group of equally prominent luminaries in EUS demonstrated a 40% “find” rate when endoscopic ultrasound was added to a traditional upper endoscopic exam (EGD)—conditions that would not have been discovered with the standard EGD alone. (EUS to detect evidence of pancreatic disease in patients with persistent or nonspecific dyspepsia: Anand Sahai,, Girish Mishra, Ian Penman, David Williams, Michael Wallace, Neven Hadzijahic, Andrew Pearson, Annette VanVelse, Brenda Hoffman, Robert Hawes, Gastrointest Endosc 2000;52:153-9). The following year they demonstrated that this strategy would be cost-cutting by virtue of eliminating other expensive tests (and thereby shortening the time to diagnosis) (Endoscopy 2001; 33: 662–667). In the ensuing 25 years these findings have been duplicated in a number studies. To translate into the real world, if you had abdominal pain and the plan was try to find an explanation by putting you to sleep, putting a scope through your mouth and into your stomach- would you prefer an exam that looked at the surfaces of the esophagus, stomach and small intestine (EGD); or one that, in addition, evaluated surrounding structures (pancreas, gall bladder, bile ducts, liver and more) as well (we call this “echo-EGD). The answer seems obvious, and the data support it. The reason this approach has not been adopted, despite the data, is access. The cost of legacy EUS equipment (~$450k and up) has limited the availability of equipment everywhere, but in particular in ASC’s (surgicenters) where the majority of these patients receive their care. For reference, ~6M “diagnostic” EGD’s are performed in the US annually- our target. b. Pancreatic cancer: is the 3rd leading cause of cancer death in the US and the 5 yr survival rates have only improved from 5-12% over the last twenty years. Among the reasons is that the disease is discovered late. EUS is the most accurate way to evaluate the pancreas (better than CT and MRI) and can identify (and biopsy) small tumors. Further, fatty infiltration of the pancreas, which may predispose to cancer can be identified by EUS. Wider screening and earlier detection are the key to better survival. c. Liver disease: metabolic or “fatty” liver disease is epidemic in the US (25% of our 340M population with fatty liver, 25% of them with liver disease). It is now the most common indication for liver transplantation in the US. EUS is an excellent (though not the only) way to evaluate the liver. Early detection means early treatment and better outcomes. 2. EVS vs Legacy systems- The advantage of EndoSound’s EVS over legacy systems is not one of visualization (although the EVS provides a superior “EGD” view”), but one of access. The EVS converts the EGD scopes found in every GI lab into EUS capable endoscopes at a fraction of the cost- solving the problem of access.
